[18F]AlF-FAPI-74 PET/CT for preoperative assessment of the peritoneal cancer index and comparison with MRI-based, surgical and pathological assessment in colorectal cancer patients eligible for CRS-HIPEC: study protocol for a prospective observational proof-of-concept study.

The TROMPET study is a prospective observational proof-of-concept trial enrolling 25 colorectal cancer patients with suspected or confirmed peritoneal metastases who are candidates for cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC), designed to determine whether [18F]AlF-FAPI-74 PET/CT can non-invasively quantify the peritoneal cancer index (PCI) with accuracy comparable to histopathological analysis of all resected lesions. Currently, reliable PCI assessment requires invasive diagnostic laparoscopy on top of CT and MRI, because standard imaging frequently misses peritoneal spread, leading to unnecessary open-close surgeries and early disease recurrence after CRS-HIPEC. FAPI-PET/CT exploits tracer binding to fibroblast activation protein overexpressed in the tumour microenvironment, potentially offering a more sensitive and quantitative non-invasive alternative. If the PCI scores derived from FAPI-PET/CT correlate strongly with pathology-confirmed scores, this protocol will form the foundation for validating FAPI-PET/CT as a standardised preoperative patient-selection tool for CRS-HIPEC.

A case report published in Clinical Nuclear Medicine describes the imaging characteristics of a rare primary extraosseous Ewing sarcoma located in the abdominal cavity and retroperitoneum, a site that is exceptionally uncommon for this tumour type. The mass exhibited only mild uptake on standard 18F-FDG PET/CT but demonstrated avid uptake on 68Ga-FAPI-04 PET/CT, highlighting a striking divergence between the two modalities. This finding suggests that 68Ga-FAPI-04 PET/CT may be a superior imaging tool for detecting and characterising retroperitoneal Ewing sarcoma when FDG avidity is low, with potential implications for staging accuracy and treatment planning in this difficult-to-diagnose malignancy.

Researchers have developed TT1-4PYR, a self-assembling Janus dendron-modified phthalocyanine nanoparticle that achieves deep penetration and selective accumulation within peritoneal metastasis lesions of gastric cancer, enabling potent photodynamic therapy (PDT). The compound harnesses an ascites-derived protein corona and macropinocytosis-mediated cellular internalization to overcome the formidable delivery barriers of the peritoneal tumor microenvironment, producing amplified reactive oxygen species (ROS) with improved tumor-targeting specificity. Comprehensive evaluation in cell culture, mouse models, and human-derived gastric cancer organoids confirmed that TT1-4PYR effectively suppresses tumor progression while maintaining a favorable biosafety profile with no systemic toxicity. These findings establish TT1-4PYR as a promising deep-penetrating photosensitizer for peritoneal metastasis treatment, with potential applicability to other metastatic malignancies.
