Clinical and ultrasound characteristics of uterine perivascular epithelioid cell tumors.

A retrospective multicenter study of 13 patients with histologically confirmed uterine PEComas found that initial transvaginal ultrasound correctly suspected malignancy in only 4 cases (31%), revealing a critical gap in preoperative detection of these rare tumors. The tumors — 8 malignant and 5 benign — most commonly appeared as solitary, round, solid masses with regular margins (62%), inhomogeneous echogenicity (69%), and moderate to rich vascularization (color score 3–4) in 9 cases (69%), with a median diameter of 42 mm (IQR 23–66 mm). Conducted across two Italian academic oncology centers using standardized MUSA terminology, the study found tumors arising from the uterine corpus (46%), cervix (38%), and isthmus (15%), and notably, shadowing and hyperechoic islands — hallmarks of uterine leiomyomas — were universally absent. These findings suggest that the combination of marked vascularity, inhomogeneous echogenicity, and absent leiomyoma-like features should raise clinical suspicion for PEComa even when margins appear regular.

Sintilimab‑related cystitis/ureteritis in high‑grade endometrial stromal sarcoma: A case report and structured literature review.

A 44-year-old woman with high-grade endometrial stromal sarcoma developed severe sintilimab-induced cystitis and ureteritis after three cycles of sintilimab combined with nab-paclitaxel and carboplatin — a rare urological immune-related adverse event that resolved dramatically with methylprednisolone and, following identification of a concomitant Enterococcus faecalis infection, with the addition of piperacillin-tazobactam. Upon re-challenge with sintilimab only mild urinary symptoms occurred, and subsequent switching to tislelizumab resulted in no recurrence of urinary complaints. This case report, supported by a structured literature review of immunotherapy-related cystitis and ureteritis cases, offers clinicians a practical reference for diagnosing and managing immune checkpoint inhibitor–induced urological toxicity and for guiding subsequent anti-tumor treatment decisions.

Measurement of Treatment Adequacy Influences Racial Disparities Estimates in Endometrial Cancer Care.

Black patients with endometrial cancer consistently received guideline-concordant treatment (GCT) at significantly lower odds than White patients across all three major U.S. databases examined, with adequacy-based odds ratios of 0.84 (95% CI 0.79–0.90) in SEER-17, 0.75 (95% CI 0.73–0.77) in the National Cancer Database, and 0.73 (95% CI 0.62–0.85) in SEER-Medicare. Researchers compared adequacy-based GCT definitions — incorporating treatment receipt, timing, amount, and duration — against simpler receipt-based definitions, finding that adequacy-based measures revealed larger racial and ethnic disparities within both NCDB and SEER-Medicare. The study demonstrates that the method used to measure treatment quality substantially shapes estimates of racial inequality in endometrial cancer care. The authors conclude that adopting adequacy-based treatment definitions provides a more comprehensive picture of care inequities and should inform future cancer disparities research, quality benchmarking, and health policy.
