Inferior Survival in Therapy-Related Chronic Myelomonocytic Leukemia: A Population-Based Analysis.

A population-based analysis of 8,432 patients with chronic myelomonocytic leukemia (CMML) diagnosed between 2000 and 2022 found that therapy-related CMML (t-CMML) is associated with significantly worse survival than de novo disease, with a median overall survival of 14 months versus 19 months and an adjusted hazard ratio for death of 1.20 (95% CI 1.09–1.32). Among the cohort, 594 patients (7.0%) met the operational criteria for t-CMML, defined by prior chemotherapy and/or radiotherapy exposure identified through the SEER registry. Inferior survival was driven specifically by patients with prior chemotherapy exposure, while those treated with radiotherapy alone did not show significantly worse outcomes compared with de novo CMML, and direct comparisons between chemotherapy-exposed and radiotherapy-only patients revealed no statistically significant difference in overall survival.

A study published in Haematologica used single-cell RNA-level analysis to map distinct aggressive and favorable cellular states within chronic myelomonocytic leukemia (CMML), a rare blood cancer that straddles the boundary between myelodysplastic syndromes and myeloproliferative neoplasms. By resolving the molecular identity of individual leukemic cells, the researchers uncovered previously hidden intra-tumor heterogeneity that associates with differences in disease behavior and clinical trajectory. These findings provide a new cellular framework for understanding why CMML patients experience widely varying outcomes and may support the development of state-specific therapeutic strategies.

A new report in the International Journal of Dermatology describes a case in which sequential neutrophilic dermatoses — inflammatory skin eruptions driven by abnormal white blood cells — appeared as early heralds of Chronic Myelomonocytic Leukemia (CMML), an MDS/MPN overlap malignancy. Immunophenotyping revealed a shared myelomonocytic cell signature present in both the skin lesions and the bone marrow, suggesting that the cutaneous infiltrate reflected the underlying clonal neoplastic process. These findings highlight that dermatologists and hematologists should consider CMML in patients presenting with recurrent or sequential neutrophilic dermatoses, as skin manifestations may precede the formal hematological diagnosis and provide an early diagnostic window.
