Fetal Craniopharyngioma: A Rare Prenatal Diagnosis.

Researchers report a prenatal diagnosis of adamantinomatous craniopharyngioma (ACP) detected at 30 weeks of gestation — one of fewer than 20 such cases ever documented in the medical literature. Routine third-trimester ultrasound revealed a midline suprasellar brain mass with mixed solid and cystic components and calcifications, subsequently confirmed by fetal MRI; after the parents opted for termination, fetopathological examination at 31 weeks confirmed the ACP diagnosis histologically. Notably, all reported fetal cases correspond exclusively to the adamantinomatous subtype and share consistent radiological and histological features, supporting the concept that the full morphological phenotype of ACP is already established during fetal life. This case highlights the importance of thorough prenatal imaging and fetopathological examination, and reminds clinicians that despite its benign classification, ACP carries serious risks for long-term visual, endocrine, and neurodevelopmental morbidity.

Relationship between retinal nerve fibre thickness, suprasellar extension and post-operative visual outcome in pituitary macroadenoma.

A prospective study of 38 pituitary macroadenoma patients found that thicker pre-operative retinal nerve fibre layer (RNFL) significantly predicted better post-operative visual recovery, with each additional 10 µm of global RNFL thickness associated with a 0.13 improvement in logMAR visual acuity (β=-0.013, P<0.001) and a 0.83 dB improvement in visual field mean deviation (β=0.083, P=0.008). Surgery produced significant improvements in both best-corrected visual acuity (median logMAR from 0.301 to 0.176, P<0.001) and visual field mean deviation (from -10.04 to -3.91 dB, P<0.001), while patients with normal pre-operative RNFL achieved substantially better post-operative visual fields (median MD -2.35 dB) compared to those with thinned RNFL (median MD -6.63 dB, P=0.009). Critically, suprasellar extension of the tumour — rather than overall tumour volume — was the anatomical parameter most strongly correlated with RNFL thinning across all sectors, offering clinicians a practical imaging and OCT-based framework for predicting visual prognosis before surgery.
