A Novel Technique of Traction Pre-Bladder Suturing in Single-Position Robot-Assisted Nephroureterectomy: Simplifying the Management of the Intramural Ureter.

A novel four-step traction pre-bladder suturing technique for robot-assisted laparoscopic nephroureterectomy (RALNU) proved feasible and safe in all 10 consecutive patients with early-stage upper tract urothelial carcinoma (UTUC) treated between October 2022 and August 2023. The procedure achieved a mean operative time of 145 minutes — with 38 minutes dedicated to managing the intramural ureteral segment — and a mean blood loss of just 35 mL, with no intraoperative conversions or major complications. Pathological analysis confirmed complete (R0) resection of low-grade noninvasive carcinoma in every patient, and two metachronous bladder recurrences were observed over a median 12-month follow-up period. The technique, which combines mucosal tenting, a purse-string containment suture, en bloc mucosal resection, and reinforced double-layer bladder closure, offers a promising approach to minimise urine spillage and ensure secure bladder reconstruction during RALNU.

A scoping review of 38 studies found that Specificity Protein 1 (Sp1), a zinc-finger transcription factor, is consistently overexpressed in oral squamous cell carcinoma (OSCC) and associated with advanced clinical stage, lymph node metastasis, and poor prognosis, although direct human-tissue evidence was derived from only two studies. Single-cell RNA sequencing analysis of 5,865 cells from 18 tumors confirmed that Sp1-associated transcriptional programs drive epithelial-mesenchymal transition, immune evasion, and oncogenic signaling across distinct tumor cell compartments. Multiple natural and synthetic agents were shown in xenograft and cell-line models to suppress Sp1 activity and induce apoptosis, yet despite this substantial preclinical body of evidence, no Sp1-directed therapy has advanced to clinical testing. The authors identify a critical translational gap and advocate for biomarker-driven combination strategies to convert preclinical promise into meaningful benefit for patients with oral cancer.

A retrospective case series from Lebanon's LAU Medical Center Rizk Hospital found that trametinib, a MEK inhibitor, produced clinical improvement in all four pediatric patients with progressive low-grade gliomas (LGGs) harboring distinct molecular drivers (KIAA1549-BRAF, NF1 Loss, and BRAF V600E), with one patient also achieving a partial radiographic response over a median treatment duration of 30.2 months (range 22–35.5 months). The drug was obtained on a compassionate-use basis through an international Managed Access Program, representing the first published report on targeted therapy use in Lebanon and one of very few from low- to middle-income countries (LMICs). Side effects were manageable, consisting primarily of skin reactions and gastrointestinal complaints with only rare therapy interruptions, while quality-of-life assessment using the Pediatric Quality of Life Inventory 4.0 revealed that physical and school functioning were the most affected domains. These findings suggest that MEK inhibitors such as trametinib may be a feasible treatment option for selected pediatric LGG patients in LMIC settings where appropriate diagnostic and monitoring infrastructure is in place.

A population-based study of 62 pediatric patients with primary mediastinal large B-cell lymphoma (PMBCL) identified pretreatment plasma cytokine profiles that distinguish tumor burden from prognosis, with IL-6 showing the strongest link to outcome — a hazard ratio of 6.8 (95% CI, 1.5–30.9) for inferior event-free survival. CCL17 levels were dramatically elevated in PMBCL patients compared to 26 age-matched controls (median 1695 pg/ml vs. 81 pg/ml; p<0.0001), and CCL17, CXCL9, and CXCL10 correlated significantly with mediastinal tumor volume and lactate dehydrogenase activity, though not with survival. In a uniformly treated subgroup of 50 patients receiving dose-adjusted EPOCH with rituximab, elevated IL-6, IL-10, IL-17A, and IL-22 each predicted inferior event-free survival, establishing a panel of novel biomarkers that may guide risk stratification in this lymphoma subtype currently lacking established prognostic markers.

A comprehensive review finds that cancer cells rapidly develop resistance to protein kinase inhibitors (PKIs) such as imatinib and osimertinib through genetic mutations that block or eliminate drug-binding sites — most notably the BCR::ABL1 T315I mutation relevant to myeloproliferative neoplasms and the EGFR T790M mutation in lung cancer. These resistance mechanisms have driven the iterative design of second- and third-generation inhibitor therapies intended to overcome each successive barrier. Emerging technologies including single-cell sequencing, CRISPR screens, and liquid biopsies now offer clinicians more precise tools to detect and combat resistance in real time. The authors also stress that equitable access to these therapies, reduced drug costs, and international collaboration are essential for translating scientific advances into meaningful patient outcomes.
