Reduced-Dose or Dexamethasone-Free Netupitant/Palonosetron Plus Olanzapine for Chemotherapy-Induced Nausea and Vomiting

A phase III noninferiority trial published in the Journal of Clinical Oncology demonstrated that reducing or eliminating dexamethasone from the antiemetic regimen of netupitant/palonosetron combined with olanzapine maintains adequate control of chemotherapy-induced nausea and vomiting (CINV) in patients receiving highly emetogenic chemotherapy (HEC). This is clinically significant because dexamethasone, while effective as an antiemetic, carries substantial side effects including insomnia, hyperglycemia, and immunosuppression that impair quality of life. Establishing noninferiority of a steroid-sparing regimen could change standard supportive-care practice for a broad population of cancer patients undergoing HEC.
