A large reconstructed individual patient data meta-analysis of phase 3 randomized trials published in The Lancet Oncology confirms that adding CDK4/6 inhibitors to endocrine therapy significantly improves both progression-free survival and overall survival in patients with hormone receptor-positive, HER2-negative advanced breast cancer. Crucially, the analysis reveals clinically meaningful differences between agents: ribociclib and abemaciclib demonstrated statistically significant overall survival benefits, whereas palbociclib did not. These findings may inform treatment selection among the three widely available CDK4/6 inhibitors and could influence clinical guidelines. The authors call for head-to-head trials and further evaluation of toxicity profiles and patient-reported outcomes to guide personalized treatment decisions.
